Our quick and easy physical helps commercial drivers fulfill regulatory, department of transportation requirements.
What to expect at your visit
1 Wear comfortable clothes and bring any required forms, including the DOT Medical Examination Report with "Driver Information" and "Health History" sections completed. Sign in at the electronic kiosk to see a practitioner.
2We will perform an examination, including:
- Review of your health history
- Urine test
- A physical exam
3We willl complete your Medical Examination Report for Commercial Driver Fitness Determination form, provide a summary of your exam and a Medical Examiner’s Certificate as appropriate.
4We also can send the summary to your primary care provider at your request.
5Please note that additional charges apply for labs and tests. Visit our

What to know before your visit
- Service is not available in Massachusetts.
- A DOT physical should not replace an annual wellness physical with a primary care provider
- Insurance is not accepted for physicals, and payment is due at time of visit.
