What to expect at your visit
1Sign in at the clinic electronic kiosk.
No appointment necessary.
2We will perform the following:
- Blood pressure check
- Weight assessment
- Health assessment
- Blood sugar or A1c test*
- Cholesterol test*
*We will take a blood sample from your finger to perform these tests.
3We will review your results and provide
recommendations that can help you reduce your risk for chronic disease.
4We will give you a visit summary and
send it to your primary care provider at your request.

What to know before your visit
- Service is not available in Massachusetts.
- Patients must be 18 years of age or older.
- We recommend that you fast 8 to 12 hours before coming in for this
service.
