Diabetes monitoring $79

Diabetes, which affects nearly 8% of the population, is a leading cause of death in the United States – more than breast cancer and AIDS combined. Left uncontrolled, diabetes can lead to complications that affect nearly every organ in the body.

For those diagnosed with Type 1 or Type 2 diabetes, we are your support system in between visits to your primary care provider, ready and able to help manage the self-care component of your disease.

What to expect at your visit

1 Sign in at the electronic kiosk. No appointment necessary.
2We will review your medical history as well as perform an exam and tests based on your individual needs, following nationally accepted guidelines for standards of care. They may include the following:
  • A1c test
  • Blood pressure check
  • Body mass index (BMI)
  • Cholesterol test
  • Comprehensive foot exam
  • Microalbumin test (kidney function)
  • Review of lifestyle factors

4We will discuss your results and provide recommendations to keep your diabetes under control. We can also answer any questions you may have about your condition.
5 We will give you a visit summary and send it to your primary care provider at your request.

What to know before your visit

  • Service is not available in Massachusetts
  • Patients must be 18 years of age or older.
  • We recommend that you fast 8 to 12 hours before coming in for this service.
