While your primary care provider directs and guides your condition management plan, our practitioners are here to help in between visits by performing routine tests, reviewing the results, and providing educational support for those diagnosed with diabetes, high cholesterol, high blood pressure or asthma. After each visit, we can even send a record to your primary care provider to ensure continuity of care.
